The Wellness Program Requirements for 2015
Designate a Primary Care Physician. Complete a Health Risk Assessment Form, review with your Primary Care Physician and both of you sign off on the review. Consequences of failure to complete the Wellness Program Requirements for 2015 by October 2015. $50 Premium subsidy for each participant who fails to complete the requirements, spread out over the pay periods beginning January 2016. Participants must complete Recommended Healthy Activities or Disease Management Program. Consequences for failure to complete Wellness Program Requirements for 2016. $250 Premium subsidy for each participant who fails to complete the requirements, spread out over the pay periods beginning January 2017. Increases to $375 in 2018.
