STATE OF MARYLAND EMPLOYEE/RETIREE HEALTH BENEFITS FOR 2015

WHO IS ELIGIBLE FOR THE WELLNESS PROGRAM.   Active Employees and Spouses.        Non-Medicare eligible Retirees and Spouses

WHO IS NOT ELIGIBLE FOR THE WELLNESS PROGRAM.  Dependent children - regardless of age.    Retirees eligible for Medicare and their spouses.   Spouses eligible for Medicare.

The Wellness Program Requirements for 2015

         Designate a Primary Care Physician.   Complete a Health Risk Assessment Form, review with your Primary Care Physician and both of you  sign off on the review. Consequences of failure to complete the Wellness Program Requirements for 2015 by October 2015.  $50 Premium subsidy for each participant who fails to complete the requirements, spread out over  the pay periods beginning January 2016.  Increases to $75 for 2017.          Participants must complete Recommended Healthy Activities or Disease Management Program. Consequences for failure to complete Wellness Program Requirements for 2016.         $250 Premium subsidy for each participant who fails to complete the requirements, spread out  over the pay periods beginning January 2017.  Increases to $375 in 2018.
